Breach of condition / undertaking
The Board referred allegations that the Practitioner contravened a condition of his registration as a pharmacist by dispensing diazepam to a patient and then made a false and/or misleading record in relation to same.
The Panel found that the Practitioner had behaved in a way that constituted unprofessional conduct.
The Panel reprimanded the Practitioner and imposed conditions on the Practitioner's registration requiring him undertake education in relation to the legal, ethical and professional responsibilities of pharmacists.
